Responsibilities of RN Case Manager:

Delivers care in alignment with the guidelines outlined in the state's nurse practice act.
Conducts initial assessments and routinely reassesses the client’s nursing requirements.
Prepares and compiles clinical documentation, progress updates, and care summaries.
Serve as backup for Critical Care Clients, In-School Staffing & PDN.
Ensures accurate and thorough maintenance of clinical records, adhering to local, state, and federal regulations.
Provides guidance and training to care-giving staff, including supporting clinical onboarding for new hires as needed.
Participate in monthly clinical on-call rotation.
No OASIS work.

RN Case Manager Requirements:

Current unrestricted RN license in Minnesota.
Certified CPR
Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)
Minimum two (2) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred.
Critical Care Experience (1-2 years)
Reliable car/auto insurance.
Clean background and criminal record.
Understands regulatory compliance with state and federal regulations.
Ability to work on-call rotation with compensation.
Expected hours: 32 per week
